When the Affordable Care Act (ACA) (2010) was created, it was the catalyst for the Centers for Medicare and Medicaid Services (CMS) to create Value-Based Purchasing (VBP). This was aimed at moving our healthcare system towards lower cost and higher quality care. “VBP is intended to create a paradigm shift of reimbursement from volume-based, fee-for-service to a value-based purchasing program predicated on the demonstration of quality of care outcomes” (Aroh, Colella, Douglas & Eddings, 2015, p.61). CMS further elaborates saying “Value-based programs reward health care providers with incentive payments for the quality of care they give to people” (CMS.gov, 2018). These changes in health care delivery forces organization and providers to focus more on preventative healthcare and helping patients understand the importance of healthy lifestyles to avoid preventable diseases rather than relying on healthcare providers to “cure” them afterwards. A major trend evolving that will impact practice in the next 3-5 years is telehealth. Over the last 2 months, telehealth has been increasingly utilized in the wake of the COVID-19 pandemic. My urgent care centers started using telehealth to screen patients prior to coming into the site. It has helped with triaging patients to the most appropriate location for their care; whether it is something that the provider can easily treat without an office visit and preventing any possible exposure for them or for the staff, or if the patient can stay at home and self-quarantine if stable because an office visit in not required, or if they need an in-person evaluation they can direct them to the Urgent Care Center or to the Emergency Department as appropriate.
APRNs will be needed to help supplement the provider to patient ratios. Top U.S. performers have provider to patient ratios at 1,030:1, in my State of Maryland, the average is 1,140:1, however for Charles County where I live and work, the ratio is 2,530:1, more than double that of the top performers.
